Self Report Module

How to Submit Self-Report Semester

Step 1 Login to YesPortal using registered account and go to menu 'Self Report'.

Step 2 Click on the 'Self Report Request' link button. Self Report form will appear. Fill it out and click 'Create'.

Step 3 After submitting the Self Report Form, system will then show self report list. You can view the status of your submission application here.


How to Check Submission Self-Report Status

Step 1 To Check for the status of a Self Report submission, click on 'Self Report'.

Step 2 If your status is 'Pending', you can change any information by clicking on the 'Edit'.

Step 3 After the results has been verified, no adjustment can be done. Your submission status will change from Pending to Submission Completed.

Step 4 Click on the 'Print Completion Letter' to download the Self Report Completion Letter.
